認知症医療における医科歯科連携の現在地点：ECCO プロジェクト

　超高齢社会の日本において，認知症は既に歯科医療従事者にとって身近な疾患の一つとなった．認知症に関する国家戦略である認知症施策推進大綱においても，歯科医師は認知症の早期発見・早期対応の一端を担うことを期待されており，認知症対応力の向上や口腔機能の管理，かかりつけ医との連携が求められている．ECCOプロジェクトが認知症専門医と歯科医師を対象として実施した意識調査では，認知症専門医，歯科医師ともに医科歯科連携の重要性を強く認識しており，両者の有機的連携の素地は十分にあることが示された．一方で，臨床の現場で実際に医科歯科連携が行われている場面はいまだに少ないことも報告された．また，認知症施策推進大綱の重要業績評価指標の一つである歯科医師認知症対応力向上研修の受講者数は2025 年の目標値の54.6%であり，他の医療職種と比べて低い値となっている．認知症者を日常生活圏域で支える社会の実現を目指すため，歯科医療従事者は今後より一層認知症に関する知識を深め，関連職種との連携強化を図る必要がある．歯科医師認知症対応力向上研修への参加や自身の地域の認知症ケアパスの把握に止まらず，多職種連携研修会や認知症疾患医療センターの主催する研修会へ参加し，市町村単位で顔の見える関係を構築するといった端緒を開くための積極的な取り組みが必要であると考えられる．

Abstract：
Dementia is a common concern among dental professionals in Japan’s super-aged society. The National Guidelines for the Promotion of Dementia Policies, expect dentists to play a role in early detection and response to dementia. The strategy involves enhancing their ability to cope with dementia, manage oral function, and collaborate with primary-care physicians. A survey conducted under the ECCO project, that targeted dementia specialists and dentists, revealed a strong recognition of the importance of medical–dental collaboration among both groups, indicating a solid foundation for their cooperation. However, instances of medical–dental collaboration in the care of people with dementia are rare in clinical settings. Moreover, dentists’ participation rate in training in dementia response, a key performance indicator in the National Guidelines for the Promotion of Dementia Policies, is 54.6% of the target for 2025, which is low compared to that of other medical professions. To support people with dementia in their everyday lives, dental professionals need to deepen their knowledge of dementia and strengthen interprofessional collaboration. Dentists should not only to participate in dementia-response training to understand the dementia care pathway in their own region but should also actively work to establish face-to-face relationships with other professionals by participating in workshops for interprofessional cooperation and workshops organized by the Medical Center for Dementia.
